3. How to Submit a DMCA Takedown Notice

• Identification of the copyrighted work: Clear identification of the copyrighted title or material claimed to have been infringed.

• Your Contact Information: Your full name, organization/company, physical address, telephone number, and official email address.
• Good Faith Statement: A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
• Accuracy & Perjury Statement: A statement, under penalty of perjury, that the information in the notification is accurate and that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
• Signature: A physical or electronic signature of the copyright owner or authorized representative.
